Cancellations with less than 6 hours notice are subject to a 50% fee for the service. 

“No-Show” and Cancellations within 1 Hour of appointment are subject to a full fee. 

These charges will be invoiced via email. 

Pre-paid appointments can not be refunded if the appointment is missed without notice or a no show.

If you missed your appointment without notice, you will be required to pay in full to rebook. 

Payment is required 24 hrs prior to appointment. If not received appointment will be canceled.

CLIENT INTAKE FORM
All new clients prior to appointment will fill out the client information sheet and will sign a release of liability form; this information will be kept confidential.

Your comfort level is very important to us and though it is customary to remove all clothing, you do not need to be nude underneath the draping you can leave as much clothing or as little clothing on as you like. Please do NOT remove the draping during session, even if the room temperature is too warm. You can ask your therapist to adjust the room temperature, but the draping MUST remain on at all times. 

INAPPROPRIATE BEHAVIOR (Verbal or Physical) WILL BE REPORTED TO THE AUTHORITIES.
